About us

Our “why” is simple. We both have families we love and we want to be present for them, for years to come.

We all have a story.

We all have moments and experiences, in life and in health, that shape who we are. Some
comfort us and some challenge us.

As founders of Elme, we are no different. Each of us has lived through pivotal life events that
fundamentally changed the course of our lives and how we think about wellbeing.

We also understand the real barriers to good health. Care that is fragmented and reactive,
focused on treating illness rather than preventing it. Difficulties accessing healthcare,
whether due to workforce shortages or the simple reality of finding time amid the demands of family, work, and life.

These barriers are real. And we feel them too.

That’s exactly what we’re changing.

Elme isn’t about creating just another health platform. It’s about creating a space where you can be proactive with your health. Where risks are anticipated, not discovered too late.

Where you have a clear plan and direction. And, most importantly, a place where you aren’t alone in your health journey. You’re supported, listened to and genuinely advocated for.

With Elme, you have a partner in health.

Michela Sorensen

(B.Med, FRACGP, Dip. Family Medicine and Paediatrics)
Co-Founder and Chief Medical Officer

Dr Michela is a General Practitioner, Co-Founder and Chief Medical Officer of Elme.

Michela completed her Bachelor of Medicine at Newcastle University in 2011. She then went on to work at Royal North Shore Hospital, Sydney, before completing her specialty training in General Practice, gaining her FRACGP in 2016. She has worked in diverse regions from metropolitan Sydney to rural and remote Australia, as well as rural Uganda.

In addition to her clinical work, Dr Michela provides expert guidance to organisations across the corporate health and media space, bridging clinical best practice with real-world application. Dr Michela has also built a trusted public profile through her work in the media landscape.

Elme was born from both professional and personal experience. Dr Michela saw too often people feeling unsupported within the healthcare system or presenting too late, a pattern reflected in her own family and, at times, her own life as a working mum. After a life-changing diagnosis, she recognised the need for a different model of care.

Elme exists to deliver supportive, proactive and genuinely personal healthcare, providing clarity, continuity and advocacy, so no one is left to navigate their health alone.

Sarah Horton

Co-Founder and Chief Executive Officer

Sarah is Co-Founder and CEO of Elme. She brings over a decade of experience in strategy and finance, alongside a deeply personal motivation to reimagine how healthcare is delivered.

Sarah began her career in investment banking, where she spent more than 12 years working with a range of businesses. But it was her personal experiences – particularly supporting her mother through chronic illness, and later becoming a mother herself – that reshaped how she thought about health.

She saw firsthand how even world-class healthcare systems are often reactive, stepping in only once something has gone wrong. Sarah founded Elme to help change that – building a proactive, personalised model of care that focuses on prevention, early insight, and long-term wellbeing.

At Elme, Sarah is focused on creating a thoughtful, evidence-led experience that empowers people to take control of their health – so they can live longer, healthier lives with the people they love.
